REGISTER
WITH THE PRACTICE?
If you live within our practice area and would like to register with us please call at the surgery to pick up the relevant forms. Please note that photo ID and evidence of residency will be required. When returning the completed registration forms please make an appointment to see the nurse to complete the registration process. When you attend for your appointment with the nurse please bring a urine sample and the new patient check form with you. You will be registering with the practice rather than an individual GP. However, you can book an appointment with a GP of your choice subject to appointments being available.
