REGISTER
WITH THE PRACTICE?
We operate an open list and can only take patients living within the practice area. If you are eligible please call at the
surgery to pick up special forms. Once at the surgery, make an appointment to
see the nurse to complete the registration procedures. When you attend for your
appointment please bring a urine sample. Although you will be registered with
the practice and will be able to see any available doctor, you may wish to record
a preference. If this should be the case please notify us on registering when
we will ensure it is recorded in your medical records.
